APPLY FOR SPAY/NEUTER VOUCHER

• Please fill out ONE certificate for EACH animal.‍ ‍

• We CANNOT issue a voucher until you have an appointment established with a clinic or veterinarian.

• Vouchers are valid for three months, from the date of request and are mailed within two days of receipt, excluding weekends and holidays

• Attention - New Requirement - Effective immediately All vouchers must be validated in person by the individual named on the voucher. That person must present a valid driver’s license matching their name at the participating veterinary clinic on the date of service. Vouchers will not be accepted if presented by anyone else . The named pet owner will forfeit the voucher and will need to reapply for a new one if they cannot attend. No exceptions.
